Transition to adulthood

Moving from children's to adult services can be challenging for young people with disabilities or special educational needs. 

The transitions team can support young disabled people and their families, enabling them to move from children's to adult services.

The team will develop a person centered transition plan with the young person, their family and other agencies, assess and review the current social care support needs of the young person, complete community care assessments for 17 year olds to establish their eligibility for services as a young adult and work with young people looked after and for whom there are safeguarding concerns.

Parents having their say

Planning for the future

The government is developing a new initiative which will see major statutory reforms to the way services are provided to disabled children and young people.

By 2014, it is proposed that every disabled person aged up to 25 years will be entitled to a single assessment of their needs.

The transition team will support and encourage the involvement of young people, their families and partner agencies, providing a more personalized service for young disabled people.
